File a Report

Report misconduct by a QDF operative. All reports are reviewed by Pre-command or Command.

Sign-in required to submit
You can fill out the form now. When you submit (or upload evidence), you'll be asked to sign in with Discord so we know who filed the report. Your work won't be lost.
Who are you reporting?

You can report more than one operative in a single report. The report will route to the forum matching the highest-ranked operative selected.

Reports without evidence are difficult to act on. Attach screenshots, chat logs, or short clips. Files are uploaded when you submit — nothing leaves your device until then.

Format: https://discord.com/channels/<server>/<channel>/<message>. Right-click any message in Discord → "Copy Message Link". The bot must share a server with the message's channel to read it.

Contact preference

If checked, QDF Command may DM you for follow-up. You'll see status updates in My Reports either way.

For DMs to reach you, you need "Allow direct messages from server members" enabled on a server you share with the QDF Discord bot. If your DMs are closed or you don't share a server with the bot, follow-up messages won't get through.
